We are required by law to maintain the privacy of your protected health information (PHI), provide
you with this notice describing our privacy practices and legal duties, and notify you following a
breach of unsecured PHI. We must abide by the terms of this Notice currently in effect.

Healthcare Operations: We may use or disclose your PHI for operational purposes such as
quality assessment and staff training.
We may also use or disclose your PHI in limited situations permitted or required by law, such as
for public health activities, law enforcement purposes, or to comply with court orders. Any other
uses and disclosures will be made only with your written authorization.
Your Rights Regarding Your Health Information
• You have the right to request restrictions on certain uses and disclosures of your PHI.
• You have the right to receive confidential communications in a certain way or at a certain
location.
• You have the right to inspect and obtain a copy of your health record.
• You have the right to request an amendment to your record if you believe it is incorrect or
incomplete.
• You have the right to receive an accounting of disclosures of your PHI made by us.
• You have the right to obtain a paper copy of this Notice upon request, even if you agreed to
receive it electronically.
Changes to This NoticeWe reserve the right to change the terms of this Notice and to make the new Notice provisions
effective for all PHI we maintain. The revised Notice will be available in our office and on our
website.
